Fellowship in Pain Medicine and Palliative Care

Objective

Participants in this fellowship will gain experience in: 1) the assessment and management of a broad range of problems related to nonmalignant pain; and 2) the comprehensive palliative care of patients with progressive incurable illness.

Description of the Department

The Department of Pain Medicine and Palliative Care at the Beth Israel Medical Center (BIMC) was inaugurated in September 1997 under the chairmanship of Russell K. Portenoy, MD. The first of its kind nationwide, this department has two clinical divisions, the Palliative Care Division and the Pain Division, as well as the Institute for Education and Research in Pain and Palliative Care. The department is staffed by an interdisciplinary group of physicians from the major specialties of neurology, physiatry, internal medicine and anesthesiology, as well as nurses, psychologists, a social worker, and others. The Institute staff includes full-time educators and research personnel.

Through the Palliative Care Division, this interdisciplinary team provides ongoing care for inpatients, outpatients and homebound patients with problems related to progressive medical illnesses of all types. Outpatient management is provided through an ambulatory practice at the Phillips Ambulatory Care Center by a consultation team and by a formal hospice home care program, the Jacob Perlow Hospice. Inpatient care is provided through admission to an 18-bed pain and palliative care unit. Through the Pain Division, treatment is offered to diverse patients with chronic nonmalignant pain syndromes. Patients with long-term pain are treated on a long-term basis in the ambulatory practice at the Phillips Ambulatory Care Center.

In addition to these clinical activities, staff participate actively in didactic case conferences, the case-based teaching program, clinical investigations and other educational initiatives.

Fellowship Program: Goals

The Department of Pain Medicine and Palliative Care is offering a fellowship program through which fellows will gain experience in: assessment and treatment of pain syndromes, both malignant (e.g., cancer, AIDS) and nonmalignant (e.g., neuropathic pain, musculoskeletal pain and headache); management of symptoms other than pain; assessment of palliative care concerns and the team approach to palliative care; comprehensive management of end-of-life issues; clinical research methods in problems related to pain, symptom control, quality of life and end-of-life issues.

Fellowship Program: Clinical Responsibilities

Fellows participating in this program will join the inpatient consultation team, round on the inpatient unit and see patients in the Phillips Ambulatory Care Center. Specific responsibilities will include:

Inpatient consultations Monday-Friday, 8:00am-5:00pm
Attendance at 2-4 outpatient sessions
Presentations at weekly rounds
Attendance at didactic conferences
Attendance at research meetings
