When the PCAD Pathway is ordered by the attending physician, the MD Order Sheet is to be completed by the attending physician or mid-level provider (e.g., house staff, NP).

Individualize orders including frequency of comfort assessments, vital signs (if ordered), etc.

Suggestions for medications used for palliation of symptoms in advanced disease management are indicated on the reverse side of the MD Order Sheet. Specify medications ordered in the section provided. Suggested medications will not be automatically activated unless an order is written for the medication needed.

Previous medications, routine labs and tests should be reviewed and rewritten as needed when the PCAD Pathway is ordered.
